ABSTRACT

Mulher de 21 anos, portadora de insuficiência aórticasevera reumatismal, submetida eletivamente a cirurgia de troca valvar aórtica por prótese metálica "Carbomedics 21", apresentou no pós-operatório intrahospitalar quadro clínico compatível com "sépsis" e acidente vascular cerebral embólico. A visualização no ecodopplercardiograma bidimensional (Eco-2D) transesofágico de vegetações e abscessos paraprotéticos, não identificados no Eco-2D transtorácico, foi decisiva na indicação da reoperação. A evolução satisfatória da paciente, no nosso entender, foi influenciada pela reintervenção precoce


A female patient, 21 years old, was submitted to surgical treatment of severe aortic insufficiency. She was doing well until the 9th post-operative day, when she presented sepsis and an embolic cerebrovascular attack. The transesophageal echodopplercardiogram showed paraprosthetic abscess and vegetations, that were not seen on the transthoracic echodopplercardiogram performed one day before. We are convinced that the findings on the echocardiogram were very important the good results obtained by the prompt surgical procedure.
